An Introduction

Gynecomastia is the enlargement of male breast tissue, and it affects more men than most people realize. It can result from hormonal shifts at puberty, weight changes, medication side effects, or genetics. Surgery combines glandular tissue excision with liposuction to restore a flat, masculine chest contour.

Our Technique

Excision + Liposuction

Most cases of gynecomastia involve both excess fat and true glandular (breast) tissue. Liposuction alone removes the fat but leaves the glandular component, resulting in a hard, visible disc beneath the nipple. Excision alone removes the gland but leaves a surrounding fat shelf.

Dr. Mata uses a combined approach — VASER-assisted liposuction to contour the surrounding fat and direct excision to remove the glandular tissue. The result is a smooth, flat chest without residual fullness or visible surgical evidence.

Understanding the Cause

Why It Happens

Gynecomastia has several common causes. Pubertal gynecomastia affects up to 60% of adolescent boys, often resolving on its own but sometimes persisting. Adult-onset gynecomastia can result from hormonal changes, medications (some blood pressure drugs, anti-androgens, and others), anabolic steroid use, or weight gain.

Surgery is appropriate when the condition has persisted for at least 12 months and hormonal causes have been addressed. Dr. Mata will review potential causes at consultation to ensure surgery is the right answer and that results will be durable.

Recovery

Most Men Overestimate the Downtime

Recovery from gynecomastia surgery is less intense than most patients expect. Most men return to desk work within a week, and the compression garment — worn for 4 weeks — is tolerable under normal clothing.

Gym activity, including lower-body lifting, typically resumes at 3–4 weeks. Chest-specific exercises wait until 6–8 weeks to protect the surgical result.

What to Expect

Every patient heals at their own pace. These milestones offer a realistic roadmap based on hundreds of procedures performed by Dr. Mata.

  1. Week 1

    Compression vest worn continuously. Pain manageable — typically less than expected. Walking encouraged. No lifting over 10 pounds.

  2. Week 2

    Return to desk work. Compression continues. Swelling significant but decreasing.

  3. Weeks 3–4

    Lower-body exercise resumes. Transition to part-time compression. Contour emerging beneath residual swelling.

  4. Weeks 6–8

    Chest exercises and full upper body training resume with clearance.

  5. Month 3–6

    Final contour settled. Scars progressively fade. Result is permanent when underlying cause is resolved.

Common Questions

01Will it come back?+

Not if the underlying cause is resolved. Glandular tissue that's removed doesn't grow back. However, significant weight gain, continued use of certain medications, or resumed anabolic steroid use can cause the condition to recur in residual tissue.

02Is this covered by insurance?+

Sometimes for teenagers with documented persistent gynecomastia and failed medical management. Adult gynecomastia is generally considered cosmetic and not covered. We can help you navigate the question if you want to pursue coverage.

03Will the scars be visible?+

Typically minimal. Incisions are usually placed at the border of the areola where the color change hides them, or in the natural fold beneath the chest. Liposuction incisions are a few millimeters and heal to nearly invisible.

04When can I lift weights again?+

Lower-body lifting at 3–4 weeks with clearance. Chest and shoulder work at 6–8 weeks. Most men are back to full training by month 2. The compression garment makes this easier than it sounds.
