Blepharoplasty is often the first surgical step patients consider. The eyes are the first place aging shows, and eyelid surgery produces some of the most dramatic refreshment for the least recovery time of any facial procedure. Upper, lower, or both — the right approach depends on your specific concerns.

Upper Blepharoplasty

Addressing Hooded Lids

Upper eyelid surgery removes excess skin (and sometimes a small amount of fat) from the upper lid. This addresses the hooded appearance that can make eyes look tired, close off the natural eye shape, or in severe cases actually obstruct vision.

The incision follows the natural upper eyelid crease, so the scar is essentially invisible once healed. Most patients return to work within a week with minimal residual bruising.

Lower Blepharoplasty

Under-Eye Bags and Hollows

Lower eyelid surgery addresses bags, puffiness, and excess skin below the eyes. Dr. Mata's preferred technique is transconjunctival — incisions made inside the lower eyelid so there's no visible external scar. Fat is repositioned or removed as needed, and excess skin can be addressed separately with a small incision just below the lash line.

For many patients, lower eyelid surgery is combined with fat transfer to restore volume in the tear trough area, creating a smooth transition from the lower lid to the cheek.

When to Consider

The Right Moment

Eyelid surgery is appropriate whenever the aging of the eyelids bothers you meaningfully and non-surgical treatments can't fully address it. Most patients are between 40 and 65, though younger patients with genetic upper lid heaviness or inherited under-eye bags may benefit too.

Non-surgical alternatives — PRP, fillers, lasers — can address early signs of eyelid aging. Once skin laxity or true fat herniation is present, surgery becomes the more effective option.

What to Expect

Every patient heals at their own pace. These milestones offer a realistic roadmap based on hundreds of procedures performed by Dr. Mata.

  1. Week 1

    Ice packs first 48 hours. Head elevated while sleeping. Bruising peaks day 3–4. Most patients comfortable without pain medication after day 3. Sutures removed at day 5–7.

  2. Week 2

    Most bruising resolved or easily concealed with makeup. Return to office work typical. Eyes may still feel slightly tight.

  3. Weeks 3–4

    Full return to most activities. Scars pink but barely visible. Any residual swelling continues to resolve.

  4. Weeks 6–8

    Exercise including swimming and contact resumes. Scars continuing to fade.

  5. Month 3–12

    Final result fully settled. Scars virtually invisible. Long-lasting refreshment.

Common Questions

01Should I do uppers, lowers, or both?+

Depends on what bothers you most. Upper blepharoplasty addresses hooded lids and tired upper eye appearance. Lower addresses bags and under-eye puffiness. Many patients benefit from both — and combining them is more efficient than doing them separately later.

02Will I have visible scars?+

Essentially no. Upper lid scars follow the natural crease and become invisible once healed. Lower lid scars — when needed — sit just below the lash line and heal to a thin line that's very difficult to see. Internal (transconjunctival) lower lid surgery leaves no external scar at all.

03Will my eyes look different?+

They should look like your eyes, rested — not different eyes. Dr. Mata's goal is restoration, not transformation. Done well, eyelid surgery makes you look refreshed without changing the fundamental character of your face.

04What about crow's feet?+

Eyelid surgery doesn't address crow's feet — those are treated with Botox or laser. Most patients combine their eyelid surgery with injectables for comprehensive eye-area rejuvenation, scheduled a few months before or after the surgery.
