Eyelid surgery (blepharoplasty) is a cosmetic procedure that modifies the shape and structure of the upper and lower eyelids. It involves making incisions along the eyelids to remove or reposition excess fat, muscle, or skin. Upper Eyelid Surgery

Upper eyelid blepharoplasty is a cosmetic surgery that removes excess skin and fat from the upper lids. During this operation, incisions are made in the natural creases of the upper eyelids to access the underlying muscle and fat. The surgeon then eliminates unwanted tissue to smooth the appearance of that area. This operation can help to improve drooping upper eyelids or skin hanging over the eyelashes.

Lower Eyelid Surgery

Lower eyelid surgery reduces bagginess, wrinkles, and dark circles under the eyes. During this blepharoplasty, the surgeon makes tiny incisions just below the lower lash lines or inside the lower eyelid. Unwanted fat is removed or redistributed, and excess skin is trimmed away. The surgeon may also reposition or tighten underlying muscle and tendon tissues. This procedure reduces puffy lower eyelids and improves the transition between the eyelids and the cheek. It creates a smoother appearance around the eyes.

Consultation & Preparation for Eyelid Surgery

Preparation for eyelid surgery begins with a consultation. During this meeting, we discuss the goals and expected outcomes of the operation to ensure that everything is clear. Dr. Mata and Dr. Benjamin evaluate the patient’s eyelids and facial features, take measurements, and recommend a customized treatment plan. They also review medical history and current medications to determine potential risks and whether the person is a good candidate.

If cleared or surgery, the patient receives preoperative instructions, such as temporarily stopping certain medications that increase bleeding risk, arranging transportation, and stocking up on cold compresses, gauze, eye lubricant, and other supplies for post-surgery wound care. Some additional tasks include getting lab tests done, avoiding alcohol and tobacco products, and planning to take at least 1-2 weeks off from work or strenuous activities to allow proper healing. Clear communication between patient and surgeon leading up to the operation date is vital for optimal preparation.

Eyelid Surgery Procedure

STEP 1: Anesthesia

Cosmetic eyelid surgery can be performed under different types of anesthesia. For the operation on the upper eyelid alone, we use local anesthesia while the patient remains awake. If the procedure involves both upper and lower eyelids, the surgeon will choose deep sedation to maximize comfort. Under deep sedation, the patient sleeps through the procedure and breathes on their own.
The surgeon makes small incisions along the natural creases and lines of the eyelid to limit visible scarring. For upper blepharoplasty, the opening is made in the fold of the upper lid, just below the lower edge of the brow. For lower blepharoplasty, the incision is made just below the lower lash line or inside the lower eyelid.
Through the small openings, the surgeon removes unwanted skin, muscle (sometimes), and underlying fatty tissue. This procedure helps to eliminate sagging skin or hooding of the eyelids. Trimming muscle and fat reduces bulges and puffiness. The amount removed depends on the patient’s needs and goals.
After removing the tissue, the surgeon closes the incisions with fine sutures. Special care is taken to do this neatly so that scarring is minimized. Steri-Strips may be used temporarily to support the healing process.

When performed skillfully, grafting can address hollowness, wrinkles, and a gaunt appearance around the eyes. Though it is a complex procedure, the long-term benefits make fat grafting an excellent complement to blepharoplasty.

Canthoplasty 
and/or Canthopexy

Canthoplasty and canthopexy are often performed in conjunction with eyelid surgery. A canthoplasty involves modifying the shape and position of the ..

lateral canthus (the outer corner of the eye where the upper and lower eyelids meet). This procedure can be done to lengthen or shorten the eye fissure, to correct drooping of the lateral canthus, or to achieve better symmetry between the two eyes. A canthopexy also addresses the lateral canthus but involves stabilizing and providing support to the outer corner of the eye using sutures that anchor it to bone or tissue. These adjunct procedures are often necessary to achieve optimal aesthetic results from blepharoplasty and to ensure the functional integrity of the eyelids after surgery.

 

Recovery After Eyelid Surgery

The healing process after an eyelid lift involves some swelling and bruising around the eyes that gradually resolves over 1-2 weeks. Vision may be blurry at first, but will improve as the eyes adjust. Cold compresses can provide some relief. With proper self-care, all discomfort will subside, and the eyes will look rejuvenated.

Our surgeon may prescribe pain medication to make the healing more comfortable. Most patients feel well enough to return to work or normal routines within a week. However, healing continues over the entire month after the operation. It is crucial 
to take time to rest and allow the body to mend from surgical procedures.

Excess Upper Eyelid Skin
