Buccal Fat Removal Surgery: Step by Step

Anesthesia

Local anesthesia is used to numb the inside of the mouth. This is done by injecting lidocaine into the buccal mucosa (soft tissue lining), where the incisions will be made. The anesthetic helps minimize discomfort during the procedure.

Incisions

Small incisions (about 0.3 inches wide) are made inside the mouth near the back teeth (molars). These openings provide access to the buccal fat pads.

Removal of Fat

The surgeon uses special instruments to tease portions of the buccal fat pads through the incisions. The amount of tissue removed depends on the aesthetic goals. This is done very carefully to avoid contour irregularities. The surgeon sculpts the remaining tissue to achieve a smooth facial appearance.

Closure

Once fat removal is complete, the doctor irrigates the area and ensures full sterility. The incisions are closed with special sutures, which gradually dissolve over time. No dressings or drains are necessary.

Is There Any Downtime With This Procedure?

Week 1

The cheeks usually have mild to moderate swelling and bruising during the first 7 days after surgery. Pain and discomfort can be managed with prescription medication. Patients are advised to stick to a soft diet and avoid hard or crunchy meals requiring extensive chewing. Ice packs can help reduce facial swelling. Patients should also avoid strenuous activity for at least 7-10 days.

Week 2

The swelling and bruising continue to diminish in the second week after surgery. Patients can usually return to work or school and resume light activity. A soft food diet is still recommended. Swelling may worsen after lengthy periods of activity, so ample rest is essential.

Weeks 3-4

Most bruising fades within 3-4 weeks. The swelling also decreases significantly, revealing slimmer facial contours. Patients typically feel comfortable returning to their routine. Some mild residual symptoms might last for several months.

Months Later

While the large buccal fat pads are permanently reduced, some patients may experience fat regrowth within the first year after surgery. However, this additional tissue is minimal and does not affect facial features.

When Will I Start Seeing Results?

You will start seeing results within one to two weeks after the procedure is completed and once the swelling begins to subside. For full results, it could take anywhere from two to four months to appreciate the final look. Overall, you can expect to see naturally defined cheeks with a slimmer facial appearance. Results are permanent once the procedure is completed.
